Your relationship with us begins with a one-hour new patient visit where you get acquainted with our experienced primary care physician and nurse practitioners, who have nearly 50 years of combined experience in treating older adults. The goal is to find and treat common conditions before they become chronic health issues. Our age-friendly integrated primary care model focuses on the 4 Ms (medication, mindfulness, mobility and what matters most).
